Chronic Disease Self-Management Program Lay Leader Training

The Chronic Disease Self-Management Program (CDSMP) Lay Leader Training allows new Leaders to facilitate CDSMP workshops. Trainees often register in pairs since all workshops are co-led by two Lay Leaders. The training is facilitated by two BCBH-SD Master Trainers and held in-person or virtually, using a videoconference platform.

Training requirements:

  1. Successful completion of two practice teaching sessions demonstrating a minimum set of core competency as observed by the Master Trainers
  2. Technology requirement for virtual training only:
    • Fast, reliable internet connection
    • Computer and webcam (no mobile devices or telephone participation)
    • Good sound quality
    • Proficiency using your equipment

Post-Training Requirements:

  • Schedule and co-lead a workshop within six months of training.
  • To remain an active leader, co-facilitate a minimum of one workshop per year.
  • Leaders must attend refresher sessions periodically to maintain certification.

Cost:

If applying as…

  • A Volunteer, not sponsored by an employer or organization, we ask that you complete the SDSU Volunteer Work Agreement* and agree to a background check* by SDSU. Registration is FREE.
  • Sponsored by your employer/organization to attend the training and become a certified Lay Leader for BCBH, your sponsoring employer/organization will need to sign the BCBH Memorandum of Agreement (MOA). Registration is $250.

*Agreements and link to submit online payment (if applicable) will be sent after training enrollment is complete.

Choose virtual or in-person training:

Full attendance is required (32.5 total training hours)

Virtual (Zoom video conference)
7 weeks, 13 total sessions

  • Week 1: session zero
  • Weeks 2–6: two sessions per week 2.5 hours each)
  • Training is limited to 16 trainees

In-Person
Four 6-hour days over no more than 2 weeks

  • Training is limited to 8–12 trainees

Refresher

This training is for already active leaders who have been unable to facilitate a community workshop, virtual workshop, or a series of 6-week conference calls with participants using mailed tool kits within the past 12 months to maintain their active status. The refresher training reviews the important processes in the programs (brainstorming, feedback, making an action plan, paraphrasing, etc.).

Leader trainees will be asked to complete pre-webinar homework and attend the live Zoom webinar with up to 12 trainees.
