Our office falls under the South Dakota Department of Health umbrella and works to improve quality of life, health, and wellbeing through effective leadership, surveillance, education, advocacy, and partnership.

Our goal is to empower communities with data and proven evidence-based strategies to implement solutions that build the foundation for long-term change.

Our vision is that all South Dakotans will live healthy lives free from the devastation of chronic disease and injury.

Chronic Disease State Plan

As we work together to prevent and lessen the burden of chronic disease and injury, the State Plan guides us in implementing prevention, health promotion, health equity, policy, system and environmental change strategies across South Dakota.

How we achieve our goals

Below are some of the ways we work with partners to make South Dakota Good & Healthy.

Prevention & health promotion

Promote health and risk factor reduction to decrease the onset of chronic health conditions:

  • Increase use of preventive services through public and provider education
  • Reduce infant mortality by promoting safe sleep, tobacco cessation, and prenatal care
  • Implement strategies that increase awareness of recommended, evidence-based preventive screening and its use
  • Change attitudes and beliefs about tobacco use by raising awareness of the dangers and promoting quitting

Health equity

Use programs and policy to build partner capacity and tailored interventions that help eliminate disparities:

  • Support innovative ways to deliver healthcare
  • Increase provider referrals to evidence-based education and lifestyle change programs
  • Assist at-risk individuals in accessing state and local services
  • Support tribal efforts to provide public health services to their members

Policy, systems & environment change

Transform communities by promoting social, environmental, policy, and systems approaches that support healthy living for individuals, families, and communities where they live, work, and play:

  • Encourage providers to use electronic technology that improves quality of care
  • Assist healthcare organizations to meet established standards for quality of care
  • Improve the overall health of children and adolescents
  • Support policy adoption that enhances healthy environments

Our partners

This site includes information from a variety of programs supported by the Office of Chronic Disease Prevention & Health Promotion, including:

All Women Count! (Breast and Cervical Cancer Control)

  • Coordinates statewide activities to promote early detection of breast and cervical cancer
  • Pays for screening tests for underinsured women who meet age and income guidelines

Avoid Opioid Abuse and Misuse Program

  • Avoid Opioid SD is a joint effort between the South Dakota Department of Health and Department of Social Services to address opioid abuse and misuse in the state as part of the South Dakota Opioid Abuse Strategic Plan.

Diabetes Prevention & Control

  • Collaborates with partners to design, implement, and evaluate a broad range of public health strategies to reduce the burden of diabetes and improve the quality of life for all persons affected by diabetes in South Dakota

Healthy South Dakota

  • Statewide effort to promote healthy eating and increased physical activity as a means to reduce overweight/obesity and chronic disease
  • Provides a single, reliable source of information on nutrition and physical activity

Heart Disease & Stroke Prevention

  • Promotes healthy communities through prevention, detection, and monitoring of cardiovascular diseases within South Dakota

Oral Health

  • Strives to improve the oral health of South Dakotans by collaborating with partners to assess access to care, workforce and dental health needs
  • Supports programs that provide education for consumers and professionals

South Dakota Cancer Registry

  • Ensures the coordination of cancer reporting in the state

SD Colorectal Cancer Control Program

  • Promotes screening and early detection of colorectal cancer
  • Coordinates efforts to increase access to colorectal screenings by combining screening with other health care activities such as annual flu shots (FluFIT) and evaluating healthcare provider knowledge.

South Dakota Comprehensive Cancer Control Program

  • Dedicated to bringing together individuals, organizations, health care providers, and agencies in order to reduce the burden that cancer places on the people of our state

South Dakota Violent Death Reporting System

  • National Violent Death Reporting System (NVDRS) is the only state-based anonymous surveillance system that covers all types of violent deaths. NVDRS collects information about the who, what, where, and how on violent deaths and provides insight about why they occurred. The South Dakota Violent Death Reporting System (SD-VDRS) began collecting data in 2019, focusing on Pennington and Minnehaha counties. Statewide surveillance began January 1, 2020.

Tobacco Control

  • Coordinates state efforts to prevent young people from starting to use tobacco products, help current tobacco users quit, and reduce nonsmokers’ exposure to second-hand smoke.
